The obligation behind UAE health insurance cost

Mandatory employer-provided cover reached all seven emirates on 1 January 2025. Abu Dhabi has required it since 2006 and Dubai since 2014, with Sharjah, Ajman, Umm Al Quwain, Ras Al Khaimah and Fujairah joining under the federal expansion.

Roughly three million additional people came into coverage through that change. No valid insurance means no residence visa, either new or renewed. That makes UAE health insurance cost a hiring blocker rather than a benefits question.

Three regulators, three prices

Northern Emirates. The federal Basic Health Insurance Scheme, administered by MOHRE, costs approximately AED 320 per employee per year. It is the cheapest compliant option in the country.

Dubai. The Dubai Health Authority’s Essential Benefits Plan runs roughly AED 550 to AED 750, with an annual coverage cap of AED 150,000. DHA-approved insurers must offer it at regulated rates, so shopping around achieves little at this tier.

Abu Dhabi. The Department of Health basic plan starts near AED 950, with coverage caps between AED 150,000 and AED 250,000 depending on tier.

The UAE health insurance cost gap nobody prices in

Abu Dhabi’s basic plan costs approximately three times the Northern Emirates equivalent for identical legal compliance. Dubai, meanwhile, sits at roughly double.

UAE health insurance cost as a share of payroll

Take an employee on the AED 6,000 monthly minimum, so an annual salary of AED 72,000.

Basic cover represents 0.44 percent of that salary in the Northern Emirates, 0.90 percent in Dubai and 1.32 percent in Abu Dhabi. Move to a comprehensive plan at AED 6,000 a year and the same benefit reaches 8.33 percent of salary.

That final number is the one founders miss. Health cover can quietly consume more of your payroll than end-of-service accrual.

The penalty arithmetic

Skipping cover is not the cheaper path. The gap runs wider than most operators assume.

Dubai fines AED 500 per month per uninsured employee. Over a year that reaches AED 6,000, while an Essential Benefits Plan costs around AED 650. Non-compliance therefore costs 9.2 times compliance.

Abu Dhabi fines AED 1,000 per month per uninsured person. Annualised at AED 12,000 against a basic plan near AED 950, that is 12.6 times the cost of simply buying the policy.

Why compliant employers still get fined

Nobody chooses this deliberately. Companies fall foul during transitions instead.

A new hire whose policy lapses between onboarding and visa issuance. An employee moving between emirates. A dependant added mid-year. Each gap accrues monthly, and the fine attaches per person rather than per company.

Understand the bands before negotiating. The jump between them is where budgets break.

Basic, AED 320 to 750. Government and select private facilities, AED 150,000 cap, designed for employees earning under AED 4,000 monthly. Maternity is covered in Dubai’s plan, with normal delivery to AED 7,000 and caesarean to AED 10,000 at 10 percent co-insurance. The Northern Emirates basic package does not cover maternity at all.

Standard SME, AED 1,500 to 4,000. Wider provider network and higher limits, which is realistically the floor for retaining professional staff.

Comprehensive, AED 4,000 to 8,000. Dental, optical, enhanced maternity, broad network access.

Premium, AED 8,000 to 25,000 and above. International coverage, zero co-pay, access to Cleveland Clinic and similar.

The twenty-person decision

Model it at company scale rather than per head. Twenty employees on Dubai’s basic plan costs roughly AED 13,000 a year. Yet the same twenty on comprehensive cover costs about AED 120,000.

That AED 107,000 gap is the actual decision, and it sits alongside the reduced Nafis salary support that landed this month. Both hit the same payroll line in the same quarter.

The dependant question that moves the number

Dependant obligations differ by emirate. That is where UAE health insurance cost genuinely diverges between locations.

In some emirates the employer’s duty stops at the employee, so dependants fall to the sponsor. In others the obligation extends to a spouse and children. Dubai dependant premiums alone run from AED 630 to AED 2,500 depending on age and category.

Four moves, in order of return.

Price your registration, not just your plan. A free zone choice made on licence cost carries a UAE health insurance cost consequence of up to three times, repeated annually for every employee.

Buy the tier your retention needs, not the tier the law requires. Basic cover satisfies the visa and satisfies nobody else. In a market where pay per worker fell last year, medical cover is one of the few benefits employees actually compare.

Audit lapse risk monthly. Because penalties accrue per person per month, a quarterly check already carries three months of exposure.

Negotiate on demographics. Group pricing responds to workforce age profile and claims history far more than to insurer choice. At the basic tier the regulator sets rates anyway, so effort there is wasted.
